FFS RN (HH)Facility: Clearwater, FLSchedule: Fee-For-ServiceJob Details: * CPR Certification* Girling Home Health is looking for a fee-for-service RN.DescriptionConducts a thorough and precise assessment of the patient's condition, functional abilities, homebound status, and environmental and social factors. Makes appropriate referrals (therapy, social work) as indicated. Instructs patients, family and/or caregiver in all aspects of patient care. Develops and implements nursing care plans and discharge plans with the involvement of the patient and the patient care team. Establishes realistic patient treatment goals. Informs Case Manager of the patient's status on a routine basis. Develops and supervises Home Health Aide Care Plans. Participates in and/or conducts case conferences, inservices, competency demonstrations/evaluations, and orientation as required. Provides on-call duties as required.Qualifications:* Free from Medicare Sanctions (required)* Current RN License (required)* Current CPR Certification (required)* Adequate means of transportation to facilitate timely visits (required)* Minimum of two years experience as an RN, with one year in Home Health (preferred)EOE/M.F.D.V.
