Cardiac Rehab NurseJob Description:The overall purpose of this job is to develop, implement and evaluate the exercise and education experiences of clients referred to Phases I, II, and III Cardiac Rehabilitation and Phases II and III Pulmonary Rehabilitation. Essential tasks include formulating a written nursing assessment and care plan on all clients; demonstrating prescribed exercises, supervising, and assisting clients with exercises, and evaluating clients responses to the exercises; counseling and education of clients and families; collaborating with all members of the health care team to coordinate client care; documenting in the client chart: providing the team leader with billing information; evaluating the program and making recommendations to the team leader; and coordinating staff responsibilities during emergency situations.Job Functions:Essential Functions:1. Demonstrates proficiency in the cardiovascular and pulmonary assessments.2. Knowledgeable of cardiovascular and pulmonary anatomy, physiology and pathophysiology.3. At the time of referral (Phase I) or orientation (Phases II or III) interviews clients, completes the appropriate forms and formulates a nursing assessment. Communicates this assessment to other staff members on an ongoing basis.4. Develops at the time of referral (Phase I) or orientation (Phases II and III) an exercise and education care plan for each client. Reviews the care plan each time the client is seen and updates as needed. Communicates the care plan to other staff members both verbally and in writing on an ongoing basis.5. Demonstrates Phase I, II, and III exercises. Supervises and evaluates client?s heart rate, rhythm, blood pressure and symptomatic responses to prescribed exercises. Communicates observations to other staff members on an ongoing basis.6. Counsels/educates client?s and their families regarding their disease process therapy and appropriate lifestyle modification. Counseling/education sessions may be done in a group or individual session. Teaches (Phase II and III) group session on an assigned basis and (Phase I, II and III) individual sessions as needed.7. Develops/revises educational outlines as assigned.8. Coordinates the client?s care by collaborating on an ongoing basis with all members of the health care team, including but not limited to physician, staff nurses, dietitians, respiratory therapists, social workers and Cardiopulmonary Rehab staff.9. Communicates with the client?s physician changes in the client?s condition at the time of occurrence or within 24 hours of occurrence depending on severity of problem.10. Communicates to the client and family members regarding the client?s progress in the program.11. Documents information in the client?s chart in accordance with hospital and department policy.12. Initiates and pursues contact with physicians to obtain Phase II referrals on appropriate Phase I candidates.13. Collaborates with the Exercise Specialist to inform patients of need to schedule annual stress tests.14. Collaborates with the Exercise Specialist to complete body composition measurements and a lipid profile (at M.D. discretion) per policy.15. Collaborates with the Exercise Specialist and Team Leader to prepare client progress reports. Reviews the progress report with the client.16. Collaborates with Exercise Specialist to schedule and complete follow-up outcomes data collection sessions.17. Coordinates staff responsibilities during Phase II and I.18. Provides the Team Leader of Cardiopulmonary Rehab with proper billing information on a daily basis in accordance with established departmental procedures.19. Coordinates staff responsibilities during emergency situations. Participates in a review of emergency procedures on a monthly basis.20. Evaluates program effectiveness through staff interaction, patient surveys and needs assessment on an ongoing basis. Make recommendations to the Team Leader of Cardiopulmonary Rehabilitation regarding program development. Registered nurse in the state of Indiana.2. Graduate of a baccalaureate program desirable.3. Two years of clinical nursing experience, six months of which are in the ICU, CCU, or IMCU areas, caring for clients with Coronary Artery Disease.4. Successful completion of a basic arrhythmia interpretation course.5. Experience in adult education desirable.6. Maintain current CRP certification7. Current ACLS certification.8.
